Who Delivers Your Assessment

NeuroSwift arranges your assessment. Spot delivers it.

NeuroSwift is not a health service provider and is not registered with the NDIS. Your assessment is conducted by SPOT4You (ABN 53 684 066 091), a registered NDIS provider, through practitioners it employs or engages.

Why we tell you this

You have a right to know who is providing your service, who holds the clinical responsibility for it, and where to go if something goes wrong. Being clear about this also lets your plan manager, support coordinator, or the NDIS follow the chain from your enquiry through to the registered provider delivering the support.

Who does what

NeuroSwift

  • Takes your initial enquiry

  • Works out what you need and whether we can help

  • Matches you with a practitioner who is available and suitable

  • Arranges the appointment time and sends your confirmation

  • Handles the payment request and administration

  • Stays in contact with you about scheduling

SPOT

  • Holds NDIS registration for the supports being delivered

  • Employs or engages the practitioner who assesses you

  • Is responsible for the clinical service and its quality

  • Is responsible for the practitioner's registration, supervision and professional conduct

  • Produces your assessment report

  • Holds your clinical records

The practitioner

The practitioner allocated to you is a qualified allied health professional registered with AHPRA / the relevant professional body]. They may be employed by Spot or engaged by Spot as a subcontractor. Either way, Spot is accountable for the service they deliver.

You will be told who your practitioner is before your appointment.

What this means for you practically

Your information. When you complete our intake form, the information you give us is shared with Spot and with your allocated practitioner so your assessment can be arranged and delivered. Our Privacy Policy explains this in full. Spot handles your information under their own privacy policy (SPOT PRIVACY URL - https://spot4you.com.au/privacy-policy/).

Your agreements. You will receive a service agreement covering the assessment. NeuroSwift's, Spot's, or a joint agreement- NeuroSwift’s. Read it alongside these pages.

Your clinical records. Your assessment report and clinical notes are held by Spot. Requests for copies go to Spot at admin@spot4you.com.au, and we can help you make that request if you would like.

Your money. Neuroswift claims and is invoiced by Spot for services provided

You can go straight to the NDIS Quality and Safeguards Commission. You do not need to raise it with us or with Spot first, and doing so will not affect your service or your booking.
