1. Who we are and what this policy covers
NeuroSwift connects people seeking allied health assessments with practitioners who can deliver them. We are not the practitioner. Assessments are delivered by Spot4You an NDIS-registered provider. This policy explains how NeuroSwift handles your personal information.
Spot handles your information under its own privacy policy, available at SPOT PRIVACY POLICY URL: https://spot4you.com.au/privacy-policy/ . Once your assessment is booked, both policies apply to different parts of your journey.
We handle personal information in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
2. What we collect
Contact and identity information
Full name, email address, mobile number, and postal address if you provide it.
Information about your needs
A description of the assessment you are seeking, the service required, your availability, and any access or communication requirements you tell us about.
Funding information
Whether you are an NDIS participant, your funding type (self managed, plan managed, or NDIA managed), your NDIS participant number if you provide it, and your plan manager's or support coordinator's contact details where relevant.
Health information
Some of what you tell us, including your disability, diagnosis, or the reason you need an assessment, is health information. Health information is treated as sensitive information under the Privacy Act and attracts higher protection. We collect only what we need to match you with an appropriate practitioner and arrange your appointment.
Payment information
Payments are processed by stripe. We receive confirmation that a payment has been made and the amount. We do not store your full card details.
Third party contact details
If you give us the contact details of a support person, guardian, nominee, plan manager or support coordinator, we collect and use those details for the purpose you gave them to us. Please tell that person we hold their details.
Website and advertising information
IP address, browser and device type, pages visited, and how you arrived at our site. See section 10.

Documents you send us
We may also receive information about you from a plan manager, support coordinator, referrer, guardian or nominee acting on your behalf, and from Spot or the practitioner in relation to your appointment.
If you begin our intake form and do not complete it, we retain what you have already submitted so we can follow up with you. If you would rather we did not, contact us and we will delete it.

If you do not give us the information we ask for, we may not be able to match you with a practitioner or arrange an assessment.
5. Consent
By submitting our intake form, you consent to us collecting the information described above, including health information, and disclosing it to Spot and to the practitioner allocated to you, for the purposes set out in section 4.
You can withdraw your consent at any time by contacting us. Withdrawing consent may mean we cannot continue arranging your assessment.
6. Who we disclose it to

SPOT4You, so your assessment can be arranged and delivered. This is the core disclosure and it happens for every booking.

The practitioner allocated to you, so they can prepare for and deliver your assessment.

Your plan manager, support coordinator, guardian or nominee, where they are involved in arranging or paying for your assessment.

Our service providers, including our form platform, email and calendar systems, payment processor and IT support. They may only use your information to provide services to us.

Where required or authorised by law, including to the NDIS Quality and Safeguards Commission, the National Disability Insurance Agency, a court, or a regulator.

Where there is a serious and imminent threat to your life, health or safety, or that of another person.
We do not sell your personal information, and we do not disclose it to third parties for their own marketing.
7. Overseas disclosure
Some of the platforms we use store data outside Australia, including in the United States and the European Union. This currently includes Typeform, Google, Meta, Wix.
Where information is disclosed overseas, we take reasonable steps to ensure it is handled consistently with this policy and the Australian Privacy Principles.
8. How we store and protect it
Information is held in access controlled cloud systems. We restrict access to staff who need it to do their job, use multi factor authentication where available, and review access periodically.
If a data breach occurs that is likely to cause you serious harm, we will notify you and the Office of the Australian Information Commissioner as required under the Notifiable Data Breaches scheme.
9. How long we keep it

Enquiries that do not proceed to a booking: 12 months, then deleted.

Records relating to a booked assessment: 7 years from your last contact with us, or longer where required by law.

Clinical records of the assessment itself: held by Spot under their retention policy, not by us.
10. Cookies, analytics and advertising
Our website uses cookies and similar technologies for analytics and advertising, including Meta Pixel, Google Analytics, Google Ads.
Advertising platforms may use this information to show you our ads and to measure their performance. We do not share information about your health or disability with advertising platforms.
You can control cookies through your browser settings. Blocking them may affect how the site works.
11. Direct marketing
We may send you information about our services. Every marketing email includes an unsubscribe link, and you can opt out at any time by contacting us. Opting out of marketing does not stop appointment related emails, which we need to send to deliver the service.
12. Accessing and correcting your information
You can ask for a copy of the personal information we hold about you, and ask us to correct anything inaccurate or out of date. Contact us using the details below. We will respond within 30 days. There is no charge for making a request, though we may charge a reasonable fee for providing access in some circumstances, and we will tell you before we do.
If we refuse a request, we will explain why in writing.
13. Accessibility
If you need this policy in another format, or need help making a privacy request, contact us and we will assist. We can provide information in large print, plain language, or by phone.
14. Complaints
If you think we have mishandled your personal information, contact our privacy contact at contact@neuroswift.solutions. We will acknowledge your complaint within 5 business days and respond within 30 days.
